North Clinic is proud to introduce a new, secure connection for our patients to
their personal health information – referred to as NC SmartChart.

Your NC SmartChart will allow you the option of communicating with your
health care team and accessing important information over a highly secure internet
portal.

How Do I Sign Up?

Registering for your NC SmartChart is easy and only takes a few minutes. You
may call us directly at 763-587-7806 and we will be happy to register you over the telephone or you may click here to access our NC SmartChart registration form.

Once completed, you may print and bring this form to your next appointment, fax the form to 763-587-7982 or mail the form to:

What Happens Next?

Once you are registered, you will receive a welcoming email from Reminders@northclinic.com. It will contain your temporary password and
instructions for logging into your new, secure NC SmartChart for the first time.

You can begin using your NC SmartChart at any time, day or night to:

• Ask a nurse a non-urgent question
• View your personal health record
• View your problem list which includes past and current diagnoses
• View your lab results
• Receive educational materials
• Receive appointment reminders
• View past and future appointment
• View past and future appointment dates and times
• Request prescription refills

You may then begin enjoying the benefits of your secure, confidential and
convenient NC SmartChart!
